Consent and Communication Form
People who rely on AAC indicate that the two most important things to them are:
1) being able to say exactly what they want to say, and
2) being able to say it as fast as possible.Communication that honors these and other values is most readily achieved through the collection and analysis of language samples.
This Word document covers both consent to collect, analyze, and use language samples and the request to use expeditious forms of communication in providing services.
